I think he can be all those things. Just because someone is mentally ill doesn’t mean they aren’t responsible for their actions (though I’d say someone who is in a deep psychosis would be an exception).

I’m with you; the kids who never reported it had some serious issues. Reminds me of the Sylvia Likens case.
